As of writing this, there were approximately twenty-five (25) nursing homes, also called skilled nursing facilities, in and around Ashtabula County, Ohio.

All of the skilled nursing facilities identified in and around Ashtabula County, Ohio accept Medicare and Medicaid.

• Skilled nursing facilities that receive funds from Medicare and Medicaid must comply with specific federal regulations. These federal regulations are contained in 42 CFR Part 483. They cover everything from what items may be charged to a resident’s funds to the specific rights afforded to every resident.

There are eleven (11) nursing homes in and around Ashtabula County which are contracted with the VA. These include, Andover Village Retirement Community in Andover, Ohio, Jefferson Healthcare Center in Jefferson, Ohio, Carington Park in Ashtabula, Ohio, Country Rehabilitation Campus at Ashtabula in Ashtabula, Ohio, Geneva Center for Rehabilitation and Nursing un Geneva, Ohio,  Pine Grove Health Center in Geneva, Ohio, Ohman Family Living at Blossom in Huntsburg, Ohio, Ohman Family Living at Briar in Middlefield, Ohio, Cortland Center in Cortland, Ohio, Burton Health Care Center in Burton, Ohio, and Concord Village Skilled Nursing & Rehabilitation in Concord, Ohio.

• Skilled Nursing facilities that contract with Veteran’s Affairs must comply with the federal regulations contained in 42 CFR Part 483 and must also meet certain additional requirements and standards set by the VA. As a condition for VA approved State Veterans Home (SVH) to receive payment of per diem, the State Home must submit to the VA Medical Center of jurisdiction for each Veteran a completed VA Form 10-10SH, State Home Program Application for Care Medical Certification and a 10-10EZ, Application for Health Benefits or 10-10EZR, Health Benefits Update Form.

According to Medicare.gov, at the time of publication, two facilities in and around Ashtabula County had recently been cited for resident harm or potential harm for abuse or neglect.

Those two facilities were (1) Madison Health Care located at 7600 S. Ridge Rd. in Madison, Ohio and (2) Cardinal Woods Skilled Nursing & Rehab Center located at 6831 Chapel Road in Madison, Ohio.

Madison Health Care is one of a chain of thirty-six (36) skilled nursing facilities run by a corporate entity named Embassy Healthcare. Embassy Healthcare’s average performance rating is 2.6 out of 5 stars. This is below the national average of 3 out of 5 stars.

Cardinal Woods Skilled Nursing & Rehab Center is one of a chain of twenty (20) facilities run by a corporate entity named AOM Healthcare. AOM Healthcare’s average performance rating is 2.6 out of 5 stars. This is below the national average of 3 out of 5 stars.
