At the time of publication, there were more than sixty (60) nursing homes, also called skilled nursing facilities, in and around Mahoning County, Ohio.

Skilled nursing facilities that receive funds from Medicare and Medicaid must comply with specific federal regulations. These federal regulations are contained in 42 CFR Part 483. They cover everything from what items may be charged to a resident’s funds to the specific rights afforded to every resident.

Sixty-Six (66) skilled nursing facilities identified in and around Mahoning County, Ohio accept Medicare and Medicaid.

Skilled Nursing facilities that contract with Veteran’s Affairs must comply with the federal regulations contained in 42 CFR Part 483 and must also meet certain additional requirements and standards set by the VA. As a condition for VA approved State Veterans Home (SVH) to receive payment of per diem, the State Home must submit to the VA Medical Center of jurisdiction for each Veteran a completed VA Form 10-10SH, State Home Program Application for Care Medical Certification and a 10-10EZ, Application for Health Benefits or 10-10EZR, Health Benefits Update Form.

There are fourteen (14) nursing homes in and around Mahoning County which are contracted with the VA. These include, Continuing Healthcare of Boardman in Boardman, Ohio, Canfield Acres LLC dba Windsor House at Canfield in Canfield, Ohio, Woodlands at Austinwoods in Austintown, Ohio, Canfield Healthcare Center in Youngstown, Ohio, Beeghly Oaks Center for Rehabilitation & Healing in Youngstown, Ohio, Aventura at Humility House in Austintown, Ohio, Greenbriar Center in Boardman, Ohio, Park Center Healthcare and Rehabilitation in Youngstown, Ohio, Austintown Healthcare Center in Youngstown, Ohio, Oasis Center for Rehabilitation and Healing in Youngstown, Ohio, Windsor Health Care Center in Youngstown, Ohio, Park Vista Nursing & Rehabilitation in Youngstown, Ohio, Heritage Manor Skilled Nursing & Rehabilitation in Youngstown, Ohio, and Masternick Memorial Health Care Center in New Middletown, Ohio.

According to Medicare.gov, at the time of publication, four facilities in and around Mahoning County had recently been cited for resident harm or potential harm for abuse or neglect.

Those four facilities were (1) Community Skilled Healthcare in Warren, Ohio, (2) Canfield Healthcare Center in Youngstown, Ohio, (3) O’Brien Memorial in Masury, Ohio, and (4) Willow Woods Rehabilitation and Nursing in North Lima, Ohio.

Canfield Healthcare Center in Youngstown, Ohio, is a part of a chain of one hundred and twenty-two (122) facilities run by a corporate entity named Communicare Health. Communicare Health’s average performance rating is 2.7 out of 3 stars. This is below the national average of 3 out of 5 stars.
